Photoshop CS2. Duplicated layer with lens blur, added layer mask, started with a gradiant and then used 30% eraser to fine tune it.

You're right about picking out the bits that should be out of focus, but I think the shift from in-focus to out-of-focus is a bit too abrupt. It makes the picture look like it's of flat miniature cut-outs instead of 3D miniature models.
